Note:  If you wish to receive a shirt or plaque, this form must be completed and donation received by August 1st each year.

    ​If you are ready to complete your donation pledge, then you need to submit pledge form and complete the payment to CASI.  Mail your CASI FOC donation check to : CASI Treasurer, c/o Mike Whitten, 129 Memory Lane, Diboll, TX 75941.  Please note on the check "FOC in name of ..........".
